If you are interested in becoming a member of Tennessee-1 Disaster Medical Assistance Team. Click here for link to download OPTIONAL APPLICATION FOR FEDERAL EMPLOYMENT. This application needs to be completed thoroughly.

After you complete the application you need to return it along with:

·         A resume or CV

·         A passport or other photo I.D

·         Copy of your current state medical license

Please mail the application package to: 7913 Old Lee Highway, Ooltewah, TN  37363. If your application packet is not complete, you will be removed from consideration for membership.

Once your application packet has been received, it will be reviewed by the TN-1 DMAT Command Staff. You will be notified in writing whether your application for membership has been accepted or declined. TN-1 is a statewide team with a limited number of positions available.
